It does exist in physical form (I have a copy. In fact two, because the author copy didn't arrive before I left for the JSM...) It was published just before the JSM (on Aug.9) but it takes a few weeks before it reaches the warehouses. Same thing with MASS4, I expect.
Yes, but that was a couple of weeks ahead and people have got copies from Amazon.
