warnings in lmer estimation after mice

Two things spring to mind:
1. I don't think your imputation respects the data hierarchy -  see p21
here:

2. You have 58% missingness in "gain" and 81% missingness in "index", which
makes me wonder if you should be doing multiple imputation at all.
